Patients must keep all scheduled appointments and have been seen by a provider for at least 6 months in order for any paperwork (e.g. FMLA, disability, etc.) to be completed. Any other paperwork requests will be processed at the provider’s discretion.
A fee will be charged for completion of paperwork depending upon the complexity/time required.
