Insurance
Major insurances accepted:
Medicare
Blue Cross Blue Shield PPO
Cigna
Aetna
Many other insurances are also accepted. It is the patient’s responsibility to confirm that their provider is in-network with their insurance. Please contact the number on the back of your insurance card to verify that your clinician is contracted with your insurance provider.
Patients must provide the office with up-to-date insurance information.
Payment
Co-payment/co-insurance and deductibles are due prior to your visit.
For self-pay patients, payment is due on the date of service. Please contact the office for costs for a specific provider.
We understand that emergencies come up, but we require 24 hours notice for any appointment cancellation. Any no-shows or late cancellations will be charged a fee of $30 that will not be covered by insurance.
Medication Refills
Prescription refills are only provided during regular business hours (Monday - Friday) and should be requested 3-5 days prior to running out.
To ensure patient safety, you must have been seen within the past 6 months for medications to be refilled.
Controlled substances will be refilled only if appointments are kept; you must have been seen in the last 3 months to have these medications refilled.
Additional Policies
If a patient has not been seen by a provider in 12 months or more, they will be considered a new patient at their next visit.
