 Insurance

 
  • Major insurances accepted:

    • Medicare

    • Blue Cross Blue Shield PPO

    • Cigna

    • Aetna

  • Many other insurances are also accepted. It is the patient’s responsibility to confirm that their provider is in-network with their insurance. Please contact the number on the back of your insurance card to verify that your clinician is contracted with your insurance provider. 

  • Patients must provide the office with up-to-date insurance information.


Payment

 
  • Co-payment/co-insurance and deductibles are due prior to your visit.

  • For self-pay patients, payment is due on the date of service. Please contact the office for costs for a specific provider.

  • We understand that emergencies come up, but we require 24 hours notice for any appointment cancellation. Any no-shows or late cancellations will be charged a fee of $30 that will not be covered by insurance.

Medication Refills

 
  • Prescription refills are only provided during regular business hours (Monday - Friday) and should be requested 3-5 days prior to running out.

  • To ensure patient safety, you must have been seen within the past 6 months for medications to be refilled.

  • Controlled substances will be refilled only if appointments are kept; you must have been seen in the last 3 months to have these medications refilled.


Additional Policies

 
  • If a patient has not been seen by a provider in 12 months or more, they will be considered a new patient at their next visit.
